In the interest of maintaining the safety and security of our customers, employees, and property, Brady’s Welding and Machine Shop, Inc. (Brady’s) will order an investigative consumer report (background report) on you in connection with your employment application.
I understand that an investigative consumer report will be requested that may include information as to my character, work habits, performance and experience, along with reasons for terminations of past employment obtained from previous employers. The types of information that may be ordered include but are not limited to: Social Security number verification; criminal and public records; verification of prior employment, drug testing results, and worker’s compensation injuries. Workers’ compensation information will only be requested in compliance with the Federal Americans with Disabilities Act and/or other applicable federal or state laws. Further, I understand that Brady’s will be requesting information concerning my driving record and/or information from various state agencies which maintain records concerning traffic offenses, accidents, etc., as well as information concerning claims involving me in the files of insurance companies. I have a right to make a written request to Brady’s Welding & Machine Shop, Inc., upon proper identification, to request the nature and substance of all information in its files on me at the time of my request, including the sources of information. I hereby consent to you or your agent obtaining the above-described information.
I authorize, without reservation, any party or agency contacted by Brady’s Welding & Machine Shop, Inc., to furnish the above-mentioned information.
I hereby authorize the procurement of consumer reports. If hired, this authorization shall remain on file and shall serve as ongoing authorization for you to procure consumers reports at any time during my employment (or contract) period. I also agree that a copy of this form is valid like the signed original. I certify that all of the personal information I provided in my application is true and correct. It is agreed and understood that any misrepresentations of information given above shall be considered an act of dishonesty. It is agreed and understood that Brady’s Welding & Machine Shop, Inc., or its agents may investigate the applicant's background to ascertain any and all information of concern to the applicant’s record, whether it is of record or not, and the applicant releases all persons named herein from all liability for and damages on account of his/her furnishing such information. I further agree to furnish such additional information and complete such examinations that may be required to complete my qualification file. This application for employment (or contract) in no way obligates Brady’s Welding & Machine Shop, Inc., to qualify the applicant.
In the event of my qualifying, I understand that false or misleading information given in my application or interview(s) may result in discharge. I understand, also, that I am required to abide by all rules and regulations of Brady’s Welding and Machine Shop, Inc.
